Radio buttons are usually used in groups, as in this example below where someone can choose from an annual salary:
$100,000+ $50,000 - $99,999 $25,000 - $49,999
For all three radio buttons, you would want to specify the same Group Name. The Group Name and Value for each radio button could look something like this:
As radio buttons only allow one option in a "group" to be selected, the collected form information would only grab the selected value.
